Thank you for making an appointment with us!

To save time on your first visit download the forms pertaining to the body part you are seeking treatment for, print and fill them out at home. Bring them in on the day of your first appointment, email them to greenwoodphysicaltherapy@gmail.com or fax them to 203-917-4798. If you are unable to complete ahead of time please arrive 15 minutes prior to your appointment time.

Please allow for at least an hour for your appointment (does not include paperwork time).

We are asking all patients to wear a mask in the clinic. Prior to meeting with the therapist you will have your temperature taken and we will ask you to wash your hands

What else to bring:

Insurance card

Driver’s License or Picture ID

Prescription (if referred by a physician)

What to wear:

Wear comfortable, unrestrictive clothing that allows the therapist access to your injured body part.
